Comprehensive community- and home-based health care model

Overview
The countries of the South-East Asia Region (SEAR) face a major challenge from increasing health care costs. As a result, accessibility to health services becomes a critical issue that SEAR countries must address. There is an ongoing trend to shorten hospital stays. Additionally, with the rising incidence of noncommunicable diseases and an ageing population, the demand for long-term and chronic care is increasing.
To ensure cost-effective care, many health interventions can be effectively delivered within the community or at home. Moreover, for most people, home is the preferred setting for receiving care. Therefore, it is imperative to extend health services beyond hospital walls, particularly for those most in need.
The Model for Comprehensive Community- and Home-Based Health Care (CCHBHC) has been developed to improve accessibility and ensure the delivery of quality community health care.
